So this is how you add PostgreSQL extensions on NixOS:

Fulcrum initial sync is extremely slow. If your computer crashes during sync you have to start all over again. Even after you finish it, if you close it incorrectly it while it writes something you are also toast.
I found that doing periodic shutdowns and running a backup is the best strategy here. And then repeat from time to time after you get it synced.
If anyone is wondering the speed in comparison ti electrs makes it totally worth the pain.
